About BluePeak Health
We started BluePeak Health to close a gap we saw across skilled nursing and assisted living communities: residents with overlapping psychiatric, neurologic, and pain-related conditions, and no coordinated way to treat them together, on-site.
Our Mission
At BluePeak Health, our clinical objective is to bridge the critical gap between geriatric psychiatry, neurological conditions, and chronic pain management within long-term care settings. Residents in skilled nursing and assisted living communities rarely experience these conditions in isolation — and we don't treat them that way either.
Our providers implement a structured, evidence-based approach to patient care. By addressing overlapping physical and psychological conditions simultaneously, we work to stabilize patient baselines, prevent acute episodes, and reduce unnecessary hospital readmissions — in partnership with the nursing and administrative staff who know each resident best.
How We Work
Care combines in-person visits and telehealth, with the mix and cadence set by each community's clinical needs, census, and regulatory requirements — not a fixed schedule applied everywhere the same way.
A single care team manages overlapping conditions together, and shares notes with facility staff and each resident's primary care provider.
Structured follow-up and direct clinician access help catch changes in condition early — before they become a hospital transfer.
